基于我国大肠癌高发的态势和众多局部晚期及复发肠癌病例的挑战，界定复杂肠癌的概念，有利于探索规范化结合个体化的多学科治疗策略。优化针对这类复杂肠癌的临床诊治策略，是提高大肠癌疗效的关键。本文旨在提出一些普遍性的原则供同道讨论，以期引发对大肠癌这一疾病的整体把握的思考，包括：平衡原则、动态原则、多维度评价体系、目标导向、慢病全程管理理念、MDT机制、如何看待指南等。提升对大肠癌这一疾病的全面认识和对病情的全程把控，才能实现当代医学模式下的肿瘤规范化、合理、适度、个体化的治疗。复杂肠癌治疗的理念充满哲学与医学思想的交融，既有方法论，也有辩证法，既有对生存的追求，也渗透着人文的关爱。

Based on the trend of high incidence of colorectal cancer in our country and the challenges of many cases of locally advanced and recurrent colorectal cancer, to define the concept of complex colorectal cancer is helpful to explore the standardized and individualized multidisciplinary treatment strategy. Optimization of clinical diagnosis and treatment strategy for the complex colorectal cancer is the key to improve the curative effect of the disease. This paper aims to put forward some general principles for the discussion in fellows, hoping to trigger thinking of the overall grasp for the colorectal cancer, which includes the principle of balance, the principle of dynamic and multi-dimensional evaluation system, goal oriented, concept of overall chronic diseases management, mechanism of MDT, how to view the guidelines and so on. Only if enhancing the comprehensive understanding of the disease and controlling the whole process of the illness, can we fulfill the standardization, reasonable, appropriate and individualized treatment of tumor in the contemporary medical model. The concept of the treatment for complex colorectal cancer is full of the integration of philosophy and medical thought, both methodology and dialectics, which is not only the pursuit of survival, but also permeated with humanistic care.
